Chronic Pelvic Pain Syndrome

Chronic Pelvic Pain Syndrome, also known as Chronic Prostatitis/Chronic Pelvic Pain Syndrome (CP/CPPS) is a poorly-understood disorder that can affect males of practically any age or socioeconomic status. Chronic Pelvic Pain Syndrome is a “diagnosis of exclusion,” meaning that all other possible causes of the patient’s pain must be ruled out before the diagnosis is made. Conditions that must be excluded include:


* chronic bacterial and/or non-bacterial prostatitis

*  bacterial, non-bacterial, or interstitial cystitis

* pelvic floor abscess/mass

* urethral stricture

* bladder neck hypertrophy


The hallmark, or “classic” marker of Chronic Pelvic Pain Syndrome is pelvic or genitourinary pain that persists for three months or longer and, as mentioned above, a definite physical cause for that pain cannot be identified.  There are suggestions, however, that multiple factors such as nervous system irritation due to an unknown agent may play a role in this condition. Symptoms of Chronic Pelvic Pain Syndrome may include:


* constant burning sensation in the penis or on urination

* pain radiating into the back or rectum

* unexplained fatigue

* depression

* pain immediately after ejaculation

* erectile dysfunction

CPPS (Chronic Pelvic Pain Syndrome)

Chronic Pelvic Pain Syndrome, or CPPS, is a condition that is marked by cycles of increasing and decreasing pain in the lower pelvis/bladder/rectum in the absence of chronic bacterial prostatitis or chronic bacterial cystitis. The pain associated with CPPS may radiate from the floor of the pelvis into the rectal area or lower back and can range in intensity from mild to temporarily incapacitating. Chronic fatigue and male sexual dysfunction are frequently component of CPPS, with painful ejaculation and erectile dysfunction being frequent complaints.


The cause of CPPS is not fully understood, but autoimmune, neurological, and hormonal factors may be significant contributors.  Treatment* of CPPS has traditionally been limited to pain management, psychotherapy directed at any co-existing psychological disorders (e.g. anxiety/panic disorders) and prescription medications such as an antidepressant (amitriptyline), NSAID (ibuprofen), gabapentin (Neurontin), or even one of the minor tranquilizers.
